Overview of Role

Please give an overview of your role and what this involves on a day-to-day basis:

• Responsible for maintaining and ensuring provision of expert clinical care for patients with multiple co-morbidities within a complex case load. • Working towards daily Quality and Outcomes Framework [QOF] targets/KPI’s. • Working autonomously and under indirect supervision to follow up and review investigations. Including the management of long-term health conditions to optimise treatment, improve quality of life and reduce hospital admissions. • Completing clinical audits to maintain continuous improvement, service development and patient outcomes. • Lead a successful quality improvement project to support patients with cardiovascular disease reduce their risk of mortality. • Working within the four pillars of advanced practice to use specialist knowledge and skills to provide independent clinical assessment, diagnosis, and treatment of patients with undiagnosed and undifferentiated conditions. • Appropriately refer patients to relevant colleagues or specialities, including but not restricted to, urgent chest Xray's, two weeks wait suspected cancer referrals and ambulatory care services.
