Overview of Role

Please give an overview of your role and what this involves on a day-to-day basis:

My role as a community sister has been both dynamic and complex. I am responsible for undertaking full holistic assessments of patients. I have gained comprehensive skills and knowledge in relation to tissue viability, catheter care, palliative care and end of life care. I am also responsible for supporting my colleagues including newly qualified nurses and students. I have gained experience in identification of risks and safeguarding concerns, by ensuring they are escalated in a timely manner and that processes are put in place to prevent harm. I attend the Gold Standard Framework meeting with local GP practices and I have stepped in for the Community Matron at Virtual Wards. I work closely with my patients to plan their care in accordance with both their needs and wishes. My role is to ensure that unnecessary hospital admissions are avoided in the best interest of patients, their families and the NHS. I am confident enough to professionally challenge poor practice in accordance with local processes should such a situation occur. I have been involved in a Quality Improvement Project in relation to syringe driver management within the community for palliative patients. In addition, I have successfully completed the physical assessment and history taking module at level 7 as well as the Mentorship course.
