I am a community assistant practitioner who supports early discharges from acute hospitals and admission avoidance patients in remaining at home and avoiding unnecessary hospital admissions.

Lots and lots of new skills - great support for my colleagues and a real MDT working approach.

Very well organised - the courses merge into the next one and fit well into learning techniques and methods -
